RESUMO

Background A large amount of iron deposition in the brain can cause neuronal damage by inducing oxidative stress, neuroinflammation, and abnormal mitochondrial function. In addition, iron deposition is also reported to be closely related to the pathogenesis of Alzheimer's disease (AD). The neurofibrillary tangles aggregated by tau hyperphosphorylation are one of the important pathological features of AD. Objective To investigate potential effect of exogenous trivalent iron ions on neuronal activity in human neuroblastoma (SH-SY5Y) cells and tau hyperphosphorylation and aggregation. Methods SH-SY5Y cells were treated with ferric chloride (FeCl3) at four concentrations (10, 100, 200, and 400 mg·L−1). Cell survival rate was then detected by CCK8 assay. Intracellular iron content was determined prussian blue (Perl's) by iron staining after 24 h exposure to FeCl3 at 10 or 200 mg·L−1. Transfection of tau-P301L plasmid was conducted to construct an AD-like cell model for tau overexpression. The differences in the expression of the phosphorylated tau (p-tau) protein in SH-SY5Y cells and SH-SY5Y cells with tau overexpression were detected by Western blotting after 24 h exposure to FeCl3 at 10 and 200 mg·L−1. After dilution with phosphate buffered saline (PBS), FeCl3, human tauR3, and FeCl3 + tauR3 were incubated at 37℃, and the fluorescence intensity reflecting tau aggregation level was measured by thioflavin T(ThT) method at 12, 24, 36, 48, 60, 72, 84, and 96 h, respectively. Meanwhile, after 96 h coincubation of FeCl3 and tauR3, the fibers formed by tau aggregation were observed under a transmission electron microscope (TEM). Results After 24 h of FeCl3 exposure, the cell survival rate of SH-SY5Y cells among all groups was statistically different (F=8.63, P<0.01). The cell survival rates in the 200 and 400 mg·L−1 groups were 80.1% and 68.7% of the control group, respectively (P<0.05). Compared with the control group, the nuclei of the 200 mg·L−1 FeCl3 group were mainly yellowish-brown after iron staining and the positive cell rate was up-regulated by 12.9% (P<0.01). After 24 h of FeCl3 exposure , the p-tau (Ser396) protein expression was statistically different among all groups (F=11.6, P<0.01). Compared with the control group, the p-tau protein expression level of SH-SY5Y cells in the 200 mg·L−1 group was up-regulated by 72.7% (P<0.01). After FeCl3-treated SH-SY5Y cells with tau overexpression for 24 h, the p-tau (Ser396) protein expression was statistically different among all groups (F=27.8, P<0.01). Compared with the tau group, the p-tau (Ser396) protein expression level of SH-SY5Y cells in the tau + 200 mg·L−1 group was up-regulated by 44.6% (P<0.05). Compared with the tauR3 group, the fluorescence intensities in the 84 and 96 h tauR3 + FeCl3 groups were up-regulated by 49.9% and 53.7% (P<0.01) respectively. After 96 h of coincubation, compared with the tauR3 group, FeCl3 + tauR3 aggravated tau aggregation and formed fiber deposition under TEM. Conclusion Exogenous trivalent iron ions may inhibit SH-SY5Y cell viability, promote the phosphorylation of tau in SH-SY5Y cells transfected with tau-P301L plasmid, and aggravate tauR3 aggregation and fiber production.

Objective:To investigate the effect and safety of intensive hyperthermia combined with low-dose cisplatin plus radiotherapy in the treatment of patients with locally advanced NSCLC.Methods:From January 2012 to December 2015, 104 patients with locally advanced NSCLC were chosen in the Second People's Hospital of Weifang and randomly divided into two groups according to the digital table, with 52 patients in each group.The control group was given low-dose cisplatin plus radiotherapy, and the observation group was given intensive hyperthermia on the basis of control group.The ORR, DCR, median OS, median PFS, KPS score, the levels of coagulation function index and tumor markers before and after treatment and incidence of side effects in the two groups were compared.Results:The DCR of the observation group was significantly higher than that of the control group(86.54% vs.69.23%, χ 2=8.24, P<0.05). The median OS and median PFS of the observation group were significantly longer than those of the control group(11.9 months vs.8.3 months; 7.5 months vs.4.7 months, t=2.56, 3.01, P<0.05). The KPS scores after treatment of the observation group were significantly higher than those of the control group and before treatment[(75.49±8.94)points vs.(68.65±7.06)points; (75.49±8.94)points vs.(62.23±6.34)points, t=2.78, 5.11, all P<0.05]. The levels of coagulation function index and tumor markers after treatment of the observation group were significantly lower than those of the control group and before treatment( t=3.14, 2.67, 3.59, 7.31; 4.89, 4.02, 4.70, 9.21; 2.44, 2.60, 3.20; 3.15, 3.78, 4.06; all P<0.05). There was no statistically significant difference in the incidence of side effects between the two groups( P>0.05). Conclusion:Intensive hyperthermia combined with low-dose cisplatin plus radiotherapy in the treatment of patients with locally advanced NSCLC can efficiently control disease progress, increase survival benefits, higher quality of life, improve coagulation function, reduce the levels of tumor markers and possess satisfactory safety.

Objective To investigate the efficacy and safety of surgical treatment for internal carotid artery atheromatous pseudo-occlusion (APO).Methods Clinical data of patients with carotid artery stenosis treated by carotid endarterectomy from Dec.,2011 to Jun.,2016 in Zhongshan Hospital Affiliated to Fudan University were analyzed retrospectively.Carotid endarterectomy were performed in 32 patients with pseudo-occlusion of the internal carotid artery (APO group).And 124 patients with traditional severe stenosis (70%-99%) served as control group.Perioperative major and minor complications,recurrence rate of ipsilateral ischemic stroke,restenosisrate and mortality in follow-up were compared between the two groups.Results Perioperative major complications:one patient (3.1 %) developed myocardial infarction in the APO group,no ischemic stroke,cerebral hemorrhage and death cases;2 (1.4%) ischemic stroke cases,6 (4.2%) myocardial infarction cases and 1 (0.7%)death case was found in control group.Perioperative minor complications:1 (3.1%) incision bleeding case,2 (6.3%) pulmonary infection cases,2 (6.3%) cerebral hyperperfusion syndrome cases were found in APO group;3 (2.1%) incision bleeding cases,2 (1.4%) incision infection cases,4 (2.8%)pulmonary infection cases,2 (1.4%) cranial nerve injury cases,2 (1.4%) cerebral hyperperfusion syndrome cases were found in control group.Patients were followed up for 6-60 months,with mean follow-up period of (35.3 ± 17.5) months.During follow-up,1 (3.1%) ipsilateral ischemic stroke recurrence case,4 (12.5%) restenosis cases,and 3 (9.4%) death cases were found in the APO group.And 8 (5.6%) ipsilateral ischemic stroke recurrence cases,9 (6.3%) restenosis cases,8 (5.6%)death cases were found in control group.There were no significant differences in perioperative major and minor complications,recurrence rate of ipsilateral ischemic stroke,restenosis rate and mortality between the two groups.Conclusions Surgical treatment for atheromatous pseudo-occlusion of the internal carotid artery is safe and effective.Perioperative and follow-up results are satisfactory.

OBJECTIVE@#Discussing the use of endoscopy in the operation of microvascular decompression of cranial nerves to treat trigeminal neuralgia (TN), hemifacial spasm (HFS), glosspharyngeal neuralgia (GN) and some tumors of the cerebellopontine angle (CPA).@*METHOD@#Since 2006, 973 cases (including 420 cases of TN, 487 cases of HFS, 66 cases of GN) had received the operation of nervous decompression by approach of postauricular suboccipital to the CPA. All cases who used the endoscopy and the material was described in detail.@*RESULT@#In 420 TN patients, 390 (92.9%) were found responsible blood vessels. Two (0.5%) were found no responsible blood vessels but arachnoid adhesion, 28 were found CPA tumor secondary trigeminal neuralgia. In 487 HFS patients, 486 were found responsible blood vessels. Between the vascular and trigeminal nerve, teflon was used to decompress and isolate the facial nerve. In 66 GN, the glossopharyngeal nerve were cut and vagus nerve were decompressed. The short-term cure rate was 100%. Cerebrospinal fluid rhinorrhea occurred in 3 cases, 1 case of cerebellar infarction, 2 patients of cerebellar hemorrhage, 2 cases of pneumatosis. All complications were cured at last. There were no deaths. All cases were followed-up for 1 to 5 years. Four cases of TN and 4 cases of HFS recurrent, and none of GN recurrence.@*CONCLUSION@#The microneurosurgery of nervous decompression endoscopy technology for the treatment of TN, HFS, GN and some tumors of the CPA is an ideally functional and etiotropic operation. It is helpful to detect the responsible blood vessel and to protect the brain tissue and nerve function with endoscope, which can improve the success rate of the operation and avoid the complications. This technology has clinical application value.

Objective To explore the influence of human resources management in emergency department on nurse's anxiety.Methods Self-Rating Anxiety Scale(SAS)was used to investigate 300 nurses in the emergency department in nine secondary level hospitals of Longgang District.Division of human resource management in these hospitals were examined and the correlation between them was statistically analyzed.Results In addition to the three hospitals,nurses anxiety status of the other six hospitals score showed different levels of anxiety,the average scores were above 50,but the hospitals,that the ratio of the three anxiety condition score was less than 50,moderate to severe anxiety were still more than 20.0%,and other six hospitals was up to 66.7%.In the human resource management to achieve excellent and qualified hospital,the anxiety rate was significantly lower than the substandard hospitals.At the same time there was close correlation between the situation of human resource management and nurses anxiety situation.Conclusions Emergency nurses generally have high anxiety,and in hospitals with poor human resource management,career anxiety is even more obvious.That is to say the anxiety of nurses is closely related to emergency department management of human resources.

RESUMO

Objective To investigate the expression and clinical significance of BRMS1 ( breast cancer metastasis suppressorl ) protein in colon cancer.Methods The expression of BRMS1 protein was detected by using EliVision immunohistochemical techniques in 46 cases of colon cancer,and adjacent non cancerous colon tissues.The clinical significance with histopathologic records was aralyzed.Results The expression levels of BRMS1 ( 34.8% ) in the colon cancer tissues was significantly lower than those of adjacent non cancerous colon tissues( x2 =23.92,P <0.01 ).The expression of BRMSl was significantly correlated with tumor size,clinical stage,and lymph node status (x2 =6.02,4.28,4.35,all P<0.01) ;BRMS1 had no correlation with age,pathological type.Conclusion BRMS1 might synergistically promote the metastasis of colon cancer.Detection of the expression of BRMS1 may be hdpful in determineing the prognosis of colon cancer.

Objective To explore prognostic factors associated with early and late recurrences after surgery for patients with gastric adenocarcinoma.Methods 214 patients who underwent curative resection for gastric adenocarcinoma were divided into 2 groups according to recurrence time:an early recurrence group included 143 patients who died within 2 years after surgery,and a late reeurrence group included 71 patients who died more than 2 years after surgery.Indexes such as age,sex,tumor site,tumor size,tumor pathology classification,Borrmann type,TNM staging,lymph node metastasis,operation type and postoperative adjuvant chemotherapy were compared.Results Single factor analysis showed that the tumor diameter,Borrmann type,TNM staging,lymph node metastasis,operation type,adjuvant chemotherapy were asoceiated with postoperative recurrence( test value were 8.32,16.21,24.90,5.22,17.23 and 3.61,respectively;all P < 0.05 ).Multifactor analysis showed that tumor diameter and TNM staging is recurrent independent factors( relative risk were 5.642 and 6.211,respectively ;all P < 0.05 ).Conclusion Tumor size and TNM staging are the most important factors for predicting the timing of recurrence after curative resection for gastric adenocarcinoma.Clinical practice should pay attention to follow up for patients with tumor diameter≥5cm,TNM Ⅲ-Ⅳ stage,especially within two years.

Objective:To summarize the clinical characteristics of 7 sporadic cases in children of influenza A (H1N1)broken out in Tianjin,and to provide clinical experience and prognosis factors for the diagnosis and prevention of this disease.Methods:The clinical data and protective measures were retrospectively analyzed in 7 children with H1N1 confirmed by Tianjin Haihe Hospital from June 9,2009 to September 21,2009,and the related literatures were reviewed.Results:There were six imported cases of H1N1 and one second-generation case.Fever and cough were the main symptom in children.There were 5 children with hyperpyrexia.Three were with expectoration.Other symptoms included anergy,dizziness,headache,nasal discharge,sneeze and pharyngalgia.Two of these children were with leukocytosis decrease,two were with infection of leukocytosis,and others were normal.Five of these children were with normal chest X-ray,one with texture disorder, and one with bilateral multiple infiltrating image.The positive influenza A virus result was detected by real-time PCR and sequencing comparison in the throat swab samples taken within 2 days from the seven children,and became negative after 5-9 days in the course of this disease.Six patients were accepted osehamivir treatment for 5 days.One was treated only with anti-virus mixture.The average length of hospital stay was 7.9 days.When the children were discharged from hospital,all the symptoms were disappeared.All the patients'families,doctors and nurses were not infected in non-directly exposed to the disease.Conclusion:The seven child patients showed mainly symptoms of fever and cough.The average course was 9.1 days,and they were all cured.Therefore,influenza A(H1N1)is preventable and curable.
